mail/Fax or e-mail the registration form found in the registration
section below to the appropriate Training Coordinator.

Registration —
Registration
will be on a first come, first served basis.
Confirmation notices will be sent out immediately after registrations are
received and when hotel reservations are made.
Cancellations must be made as soon as possible so we may fill that spot
with another participant. All
information on the registration form must be completed in order to properly
register the participant in the class requested and make the appropriate hotel
reservation.
To

requested. After the display window appears showing the form, print and fill out
the necessary information and fax to the number at the top of the form.

The
hotel cost is included in the cost of training, however, each participant is
responsible for resolving any additional expenses incurred at the hotel during their stay, including long distance telephone calls, food, room service
charges, in-room movies, etc.
Transportation —
Transportation
will be provided from and to the airport.
If needed transportation from the hotels to the training centers and back
at the end of the training day will be available also.
More specific information and directions will be included with your
confirmation letter when enrollment is made.
For
those that choose to drive, we will provide directions to your hotel and to the
training center you attend.
